SPECIALIST SERVICE ~ NURSE MAUDE:

provided services in the Canterbury region for over 100 years.

OBJECTIVES:
- To provide diabetes assessment, based on
current standards, for which the nurse is appropriately trained.
- To prioritise referrals to ensure that
acute referrals are visited on the day of referral or referred to a
competent generalist nurse.
- To maintain regular communication with the
patient's referrer: general practitioner, specialist or other diabetes
related agencies, eg: Diabetes Centre, Diabetes Life Education, Maori
Diabetes Services, Pacific Island Diabetes Services, Diabetes Christchurch
Inc, Diabetes Youth ~ Christchurch.
- To provide education and management for
clients, their families and other health professionals within the urban area
of Christchurch and to act as a resource person for outlying area.
THE SERVICE PROVIDES:
- Assessment, patient education and
evaluation of clients needs.
- Specialist diabetes management and
expertise.
- Co-ordinated care emphasising links to
appropriate support and care.
- Promotion of diabetes control and health
status improvement through appropriate management.
- Education in self management of the disease
process
- Home based visits and support for patients
and their families.
- Links and support to Maori and Pacific
Island based support programmes and communities.
- A resource for other health professionals
practising in the community.
CRITERIA FOR SERVICE:
- The client must have been diagnosed with
Diabetes Mellitus
- The support is short term with a focus on
assisting the patient towards an optimum level of independence through
education

- Specialists and the Diabetes Centre
