Diabetes
Page 1A THE DIABETES LIFE EDUCATION SERVICE:
Located at Crown Public
Health (CPH) at 76 Chester Street East,
this service offers a health promotion and public health service contracted to
Crown Public Health. It also undertakes a limited amount of personal health care
with group education for patients with Type 2 diabetes and other services, such
as "supermarket tours" assisting people with diabetes in making
appropriate food choices. Diabetes Life
staff liaise with general practitioners, Diabetes Christchurch and other groups in the community to
raise awareness of diabetes. Projects: Target
groups of people at higher risk, such as Maori, Pacific Island, Asian and other
minor ethnic groups and encourage lifestyle changes such as increasing physical
activity and eating a healthy diet.

OUT OF HOURS SERVICE:
A consultant is "on call" each week. They physician "on
call" can be reached by cell-phone via the operator. The on call
physician also carries a telepager to back up the cell-phone call system.

REFERRAL/CONSULTATION GUIDELINES/INSTRUCTIONS:
Acute hospital referrals need to state which health professional (e.g.
doctor, nurse, podiatrist) they require input from. Early discharge
planning is appropriate, as it enables provision of a better service for
the patients. Referrals should be faxed to 80171 (364-0171). Management
problems can be discussed with the "on call" diabetes physician
or (during normal working hours) the endocrinology registrar.
All
patients who need dietitian advice should be referred to the ward
dietitian. Podiatrists
at the Diabetes Centre care for patients with foot ulcers and patients at
high risk of developing ulceration. Lower risk patients requiring routine
podiatry care should be referred to Podiatry Outpatients or a community
podiatrist. The general consultation form or multi-disciplinary referral
forms can be used for referrals to the Diabetes Centre.
All
general practitioner referrals should be faxed to 03 364-0171, for one to
one consultations with the diabetes physician, nurse educator, dietitian,
podiatrist and clinical psychologist. Please state clearly which health
professional services you wish to access, (if unsure, give as much
clinical information as possible). Patients referred for the diabetes
physician may see a nurse educator and dietitian first followed by a
clinic appointment with the physician. For Group education, fax referrals

OTHER RELEVANT INFORMATION:
It is expected that management of uncomplicated Type 2 diabetes will be in
the general practice setting. The Clinic focus is on more complex patients
with Type 2 diabetes and patients with Type 1 diabetes. The Paediatric
Outpatient Department deals with all children and adolescents from 0 to 13
years of age. After this age, they are managed at the Adolescent Diabetes
Clinic at the Diabetes Centre. Specific Pacific island Clinics are also
run.
